The Clinical Pharmacist in the Population Health and Prescription Management (PHARxM) provides clinical, and educational functions as necessary to support the overall patient care provided by
PHARXMTM
Services. The Clinical Practice Pharmacist provides medication therapy management for patients as defined under medical staff approved protocols. This individual supports and contributes to process improvement within the PHARxM clinics and department and participate in the training end mentorship of Pharmacy Students and Pharmacy Residents.
Professional Duties
Follow medical staff approved protocols in management of patients defined under PHARxM department.
Provide comprehensive medication therapy consults to patients referred to PHARxM.
Collaborate with the interdisciplinary team to ensure optimal patient care coordination.
Document patient interactions and consultation's in the patient's electronic medical/health record (EMR/EHR) in accordance with established departmental policies, procedures and timeframes.
Conduct critical patient monitoring and comprehensive chart reviews to identify, prevent, and mitigate drug-related problems, including adverse drug reactions, drug interactions, medication omissions, and inappropriate use.
Conduct thorough medication histories, assess patient compliance/progress, and suggest modifications to achieve desired treatment outcomes.
Provide a consistent process of patient care that ensures the appropriateness, effectiveness and safety of the patient's medication use.
Utilize electronic health records (EHR) to document interventions and track patient outcomes.
Evaluate complex medicine regimens, identify potential interactions, and recommend evidence-based therapeutic alternatives to optimize efficacy and safety.
Make appropriate evidence-based, patient-centered medication recommendations.
Provide education to patients to promote health and wellness.
Maintain proficiency as a preceptor and actively demonstrate education and mentorship skills by developing student, resident, and staff training experiences/competencies and creating new training opportunities.
Participate in on-call schedule.
Perform other duties as assigned.

Qualifications
PharmD, BS in Pharmacy, or foreign equivalent, with a valid CA RPh license required.
Current California Pharmacist License required. Board Certification, preferred.
2-5 years of ambulatory care experience required. PGY1 preferred. PGY1 training may substitute 3 years of ambulatory care experience.
Active member of a State or National Pharmacy Organization, preferred.
Ability to utilize Microsoft Office applications (Word, Excel, Outlook, Power Point) and other computer software/applications.
Ability to effectively communicate with individuals within all levels of the organization, patients, patient family members, providers, vendors and others.
Ability to work independently and as part of a team.
Ability to manage time effectively and prioritize tasks to meet established deadlines.
BLS required within 60 day of hire. BLS not required for employees who are not involved in direct patient care.

Description The Social Services Case Manager IV (SSCMIV) is the fourth level position within the Social Services Case Manager positions and is a member of the interdisciplinary care team (IDCT). The SSCMIV will carry a designated caseload of moderate to high complexity and will case manage each member until the member's social services/behavioral health needs have been met. Will escalate cases to higher level SSCM's as deemed appropriate based on the case complexity level. Will collaborate with other staff, the patient, and the patient's family members to facilitate continuity of care, participation in company programs and proper utilization of community resources. This role will conduct visits across home, clinic, and community settings. Professional Duties Assist with scheduling appointments, obtaining referrals, and filling out forms. Assist with end of life care conversation and Durable Power of Attorney (DPOA)/Physician Orders for Life-Sustaining Treatment (POLST), VA benefits coordination, and hospice and palliative care referrals as deemed necessary. Assist with various financial, community, and government applications and forms. Determine when to involve community protective services agencies and mandated reporting. Interview patients to ascertain the nature of their problems and develop care plans with the patient and family, following up as needed. Participates in the preparation, evaluation, and implementation of the plan of care to assure that the highest quality of social services is being offered to patients/families and that the goals of continuity and comprehensiveness of care are achieved. Act as liaison between the member, their family, the IDCT, and any other appropriate parties in the DOHC system or community to assure that the necessary information is shared to facilitate well informed healthcare decisions and plan of care goal attainability, while protecting the member's healthcare information. Connect patients with resources that may include outpatient or inpatient behavioral health, substance use for detox/rehab, or other resources because of inadequate support system, lack of appropriate resources or understanding of their medical benefits. Carry a caseload of moderate to high complexity and determine the needs of clients, develop and carry out care plan for assigned case load, prepare and maintain case records. Perform treatment plan casework with a high degree of independence. Serve as a Social Services resource to other members of the IDC Team, as needed. Assist in developing and maintaining collaborative relationships with community medical/healthcare services and social services agencies. During engagement with members, address those criteria that ensure quality outcomes and completion of
DPOA/POLST.
These may include (but are not limited to): quality measures, preventative care, routine follow-up, and immunizations. These criteria may be met during PCP visits, annual wellness visits, optometry visits, and/or wellness fairs. Inform social services manager of any issues that are outside scope of practice or any issues arise. Identify and address the psychosocial needs of the member and/or family. Develop a biopsychosocial work assessment based on member's specific medical, psychological, financial, and other assessed needs. Perform other duties assigned. Serve as an ambassador for Desert Oasis Healthcare and Family Hospice Care at all times and positively shape the customer experience Help others, offering to assist (Ask, how may I help you?) Job Summary Provides Physical Therapy services within scope of practice and under the direction of a registered Physical Therapist and according to plan of care.
