Category Pharmacy - Clinical Pharmacist Job Type Direct Hire A Clinical Pharmacist opportunity is available at a respected medical center in Port Matilda, PA, offering a hybrid role blending clinical and staffing responsibilities. This position involves dedicating approximately 60% of time to clinical duties focused on Population Health and Ambulatory Care, particularly managing medication regimens for patients with chronic conditions such as diabetes. The remaining 40% involves staffing activities within the pharmacy. There is potential for this role to evolve into a fully clinical position as the clinical services program grows.
Responsibilities:
Manage medication therapies for patients within the health system, emphasizing target disease states. Collaborate with interdisciplinary teams to optimize patient outcomes. Provide staffing support as needed within the pharmacy. Assist in expanding clinical pharmacy services within the hospital setting. Utilize Meditech information systems effectively for patient care management.
Qualifications:
Licensed Pharmacist in Pennsylvania. Experience in ambulatory care or inpatient pharmacy practice. Completion of a residency program is highly desirable. Strong clinical skills and a passion for patient-centered care. Ability to work variable days and afternoon shifts, primarily 8-hour day shifts.
