Category Pharmacy - Clinical Pharmacist Job Type Direct Hire A healthcare facility in State College, PA is seeking a Clinical Pharmacist to join their team on a permanent basis, with potential for temp-to-hire arrangements. This unique hybrid role combines clinical and staffing responsibilities, offering a dynamic opportunity to impact patient care in a growing program. In this position, approximately 60% of your time will be devoted to clinical work focused on Population Health and Ambulatory Care, particularly managing medication regimens for patients with chronic conditions such as diabetes. The remaining 40% will involve staffing duties within the pharmacy. As the clinical program expands, there is significant potential for the position to evolve into a 100% clinical role.
Key Responsibilities:
Collaborate with the healthcare team to optimize medication therapy management, especially in targeted disease states Provide direct patient care in ambulatory settings and support population health initiatives Assist with pharmacy staffing and operational support Utilize Meditech for documentation and workflow management Contribute to the growth and development of clinical pharmacy services within the hospital
Desired Qualifications:
Active Pennsylvania pharmacist license Experience in ambulatory care or inpatient pharmacy settings Completion of a residency program is highly preferred Strong communication skills and commitment to patient-centered care
Location:
State College, PA Schedule:
Variable days and afternoon shifts, primarily 8-hour day shifts
Information Technology:
