This unique hybrid role blends clinical practice with staffing responsibilities—approximately 60% clinical focus and 40% staffing duties. The clinical component emphasizes Population Health and Ambulatory Care, with a focus on managing medication regimens in target disease areas such as Diabetes. The pharmacist will work closely with patients within the health system to optimize therapeutic outcomes. State College, PA Schedule:
Variable days/afternoons, primarily day shifts; 8-hour shifts

Provide clinical pharmacy services in a hybrid staffing/clinical capacity Manage patient medication regimens with a focus on chronic disease management (e.g., Diabetes) Collaborate with healthcare teams to support population health initiatives Support staffing activities and ensure operational pharmacy coverage
Qualifications/Experience:
