This role focuses on optimizing pharmacologic treatment plans in collaboration with multidisciplinary teams to ensure safe, effective, and evidence-based care across various inpatient units and crosses care to patient in the outpatient clinical space. The specialist ensures optimal therapeutic outcomes through individualized patient care and clinical leadership As an expert practitioner, educator, consultant, researcher, and clinical leader, the Pharmacotherapy Specialist contributes to high‑quality patient outcomes, advances pharmacy practice, and supports the mission, vision, and values of Tampa General Hospital.
TGH.org/TGHatHome Qualifications Doctorate Degree in Pharmacy required. Pharmacy degree (Pharm.
D) from an accredited college of pharmacy. Completion of a PGY2 pharmacy residency accredited by ASHP in relevant environment, or equivalent work experience in specialty area, required. At least 5 years of clinical practice in a hospital setting. Residency training contributes towards this requirement. Current license as a Registered Pharmacist in the state of Florida.
Essential Functions:
Manage complex medication regimens for acute and chronic conditions within assigned specialty areas. Collaborate with physicians, nurses, and interdisciplinary teams to design, implement, and optimize pharmacotherapy plans. Perform comprehensive medication therapy management, including assessment of medication appropriateness, monitoring for interactions/adverse effects, and recommending therapy adjustments. Provide evidence‑based drug information, education, and clinical guidance to healthcare staff, patients, and caregivers. Participate in interdisciplinary rounds and contribute to patient‑centered medication management decisions. Conduct medication reconciliation at admission, transfer, and discharge to ensure accuracy and continuity of care. Maintain up‑to‑date knowledge of pharmacotherapy through ongoing professional development and continuing education. Track, analyze, and report clinical outcomes and key performance indicators, including medication‑related interventions and readmission metrics. Support quality and safety initiatives such as medication use evaluations, adverse drug reaction reporting, medication error prevention, and guideline development. Precept PGY1 and PGY2 pharmacy residents and pharmacy students for clinical rotations, research, presentations, and medication use evaluations. Serve on multidisciplinary and organizational committees to advance clinical practice and system‑level initiatives. Participate in clinical research, quality improvement projects, and formulary management activities. Contribute to continuous quality improvement through development and revision of order sets, protocols, guidelines, and policies. Enhance the electronic medical record by supporting the creation of clinical decision support tools, alerts, reports, and other medication‑related content. Support transitions of care by following patients across inpatient and outpatient settings to ensure safe, coordinated medication management. This role will support our Hospital at Home Program.
