What You'll Do Clinical Services Leadership Lead and grow patient care programs including: Transitions of Care (ARMC Partnership) Medication Therapy Management (MTM) Medication Synchronization Immunization Services and Clinics Medication Adherence Programs Chronic Disease Support Programs Wellness Initiatives Population Health Programs CPESN Clinical Initiatives Direct Patient Care Provide exceptional pharmaceutical care through: Medication counseling Clinical interventions Immunizations Medication therapy reviews Adherence consultations Chronic disease support Provider collaboration Community & Provider Engagement Build relationships with physicians and healthcare providers Represent Family Pharmacy in the community Participate in health fairs and immunization clinics Expand referral partnerships Promote innovative patient care services Operational Pharmacy Practice Prescription verification Workflow oversight Team leadership Non-sterile compounding support Clinical documentation Regulatory compliance What Makes This Position Unique?
