Duties:
  • Dispense prescription medications to patients and provide medication counseling
  • Collaborate with healthcare professionals to ensure safe and effective medication use
  • Review and interpret prescriptions for accuracy and appropriateness
  • Monitor patient medication therapy for potential drug interactions or adverse effects
  • Provide clinical pharmacy services such as medication therapy management and disease state management
  • Participate in pharmacy quality improvement initiatives
  • Maintain accurate patient records and documentation
Qualifications:
  • Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy
  • Active pharmacist license in the state of employment
  • Completion of a pharmacy residency program preferred
  • Strong knowledge of acute care, pediatrics, hospital pharmacy, and critical care settings
  • Familiarity with utilization management and pain management principles
  • Proficiency in medical terminology and anatomy knowledge
  • Excellent communication and interpersonal skills
  • Attention to detail and ability to prioritize tasks effectively```
