Category Pharmacy - Pharmacist Job Type Contract This opportunity is for a Pharmacist to support a small Long-Term Acute Care (LTAC) hospital in Forest, VA. The facility has a daily census of approximately 10 patients, providing focused and meaningful patient care. This position is a contract role, starting ASAP, with an expected duration of 13 weeks or more.
Key Responsibilities:
Dispense and manage medications accurately for LTAC patients. Collaborate closely with the healthcare team to optimize medication therapy. Monitor patient progress and adjust treatments based on clinical response and safety. Ensure compliance with hospital policies, regulatory standards, and pharmacy best practices. Maintain accurate and thorough documentation of all pharmacy activities.
Qualifications:
Active Virginia Pharmacist license required. Experience in a hospital setting preferred, ideally with LTAC exposure. Strong communication skills to effectively interact with multidisciplinary teams. Detail-oriented with excellent organizational and problem-solving abilities. Ability to work independently while adhering to clinical protocols and guidelines.
