Description The clinical pharmacist I has the responsibility for the provision of high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life. Performs all duties in a manner, which promotes team concept and reflects the Sisters of Providence mission and philosophy.
Key roles include:
The ability to provide comprehensive pharmaceutical patient care. Ability to follow department/ facility/regional policies, procedures and guidelines related to medication use system, pharmaceutical care and documentation of clinical interventions. Excellent communication skills and the ability to communicate effectively in verbal and written form to key stakeholders (peers, caregivers, patients, providers). Required Qualifications:
PharmD Doctorate of Pharmacy Or Bachelor's Degree Bachelor of Pharmacy Upon hire: Washington Pharmacist License (1) Upon hire: Basic Life Support (BLS)- where applicable Notes (1) Board of Pharmacy Preceptor License - when eligible and where applicable
Preferred Qualifications:
Acute care hospital pharmacy experience, 1 year or equivalent experience. Completion of ASHP-accredited PGY1 residency. Why Join Providence?
