  • Medication Management Rheumatology Mayo Clinic Health System
  • SWWI Mayo Clinic's Department of Pharmacy and the Department of Medicine Division of Rheumatology are seeking a dedicated and skilled Rheumatology Ambulatory Care Pharmacist to join our Medication Management Services (MMS) team. This full-time position will be based out of Onalaska, WI working hours 8 a.m.

m. , Monday through Friday,through a hybrid care model, including in-person care in rheumatology clinic and telehealth services across regional practice sites. In this role, the pharmacist will build a practice within the Rheumatology Department , supporting and expanding medication access and management of patients receiving complex and high-cost rheumatologic therapies, including medication selection, initiation, monitoring, dose titration, adverse effect management, and adherence support. The primary responsibilities include providing direct patient care (both in-clinic and virtual visits) and indirect patient care (supporting medication therapy processes, assisting with patient, provider and care team inquiries). The pharmacist will support medication access, prescription capture, and related workflows, including appropriate alignment with 340B-eligible care settings and program requirements when applicable. This position also offers opportunities for administrative tasks and process improvement activities related to patient care, as well as practice development and expansion within the department. In addition to clinical responsibilities, the pharmacist will actively contribute to quality improvement initiatives , research efforts, and education programs. The pharmacist will work closely with a diverse team, including physicians , advanced practice providers , nurses , and other clinical staff, to ensure optimal pharmacotherapy for patients.
Key Responsibilities:
Document and complete billing processes for pharmacist activities. Collaborate in protocol-driven practices and/or collaborative practice agreements as applicable. Contribute to quality improvement and resource management projects. Support research initiatives within the department and clinics. Participate in precepting activities for students and residents. Contribute to continuous improvement and patient safety efforts. Assist with other departmental projects and initiatives. Collaborate with Mayo Clinic Specialty Pharmacy, Infusion Therapy Center, and insurance prior authorization teams.
Qualifications Required Qualifications:
Wisconsin Pharmacist License:
Must be licensed to practice pharmacy in Wisconsin.
Minnesota Pharmacist License:
Must be licensed to practice pharmacy in Minnesota.
Educational Requirements:
Must have a Doctor of Pharmacy (PharmD) degree or a Bachelor of Science in Pharmacy (with at least 10 years of relevant experience if holding a Bachelor's degree). Medication Therapy Management (MTM): If limited experience in Medication Management Ambulatory Care, completion of an accredited MTM certificate program is required within 12 months of hire . Minnesota Health Care Programs (MHCP): Must be registered or eligible to be a registered MMS/MTM provider through Minnesota Health Care Programs (MHCP) .
Preferred Qualifications:
Ambulatory Care Experience:
Experience in Ambulatory Care practice, especially with collaborative practice agreements (CPAs). PGY-2
Pharmacy Residency:
Completion of an accredited PGY-2 pharmacy residency in ambulatory care .
Rheumatology Care Experience:
Experience providing care to patients with rheumatologic conditions, particularly those on biologic therapies. Board Certification (BCACP): Board certification in Ambulatory Care Pharmacy (BCACP) is preferred. Skills and Experience in
