Job Description

Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered. A skilled pharmacist is needed to support a rehabilitation hospital with 30 staffed beds and an average daily census of 28 patients. This role offers a contract-to-hire opportunity, with an initial focus on weekday evening coverage and potential for full-time placement if the match is ideal.

Schedule:

Monday to Friday Primary hours 12:30 PM to 9:00 PM (negotiable) Weekend, after-hours, and on-call duties shared with pharmacy management

Pharmacy Environment:

Small, dedicated team including 2 pharmacists and 1 technician Utilize Wellsky hospital pharmacy information system Work with Pyxis automation system Focus on inpatient medication management in a rehab setting

Key Responsibilities:

Review, verify, and dispense medication orders accurately, including sterile IV compounding (non-chemotherapy) Monitor patient therapies, recommend clinical interventions, and adjust medication based on laboratory results Collaborate closely with physicians, nurses, and other healthcare professionals to optimize patient care Ensure compliance with all hospital policies and regulatory requirements Provide drug information and counseling to staff and patients Supervise pharmacy technicians and support staff Maintain accurate documentation for all pharmacy interventions Manage inventory and controlled substances securely Lead medication safety initiatives and participate in quality assurance activities Stay current on evolving pharmacy guidelines and clinical best practices

Qualifications:
