Category Pharmacy - Pharmacist Job Type Contract An established long-term acute care hospital (LTACH) specializing in critical illness recovery is seeking an experienced Pharmacist to support their multidisciplinary team in providing comprehensive pharmaceutical care. This contract position is based in Hurricane, WV, and offers an opportunity to apply clinical expertise in a specialized inpatient setting.

Hurricane, WV Responsibilities:

Perform medication reconciliation and develop accurate home medication lists using information from patients, families, and pharmacies. Provide discharge counseling to patients and their families, ensuring understanding of medication regimens. Monitor patient drug therapy for contraindications, interactions, allergies, and appropriate dosing. Offer clinical consultation and medication recommendations to physicians and healthcare providers. Participate in antimicrobial stewardship programs, including interpretation of culture results and targeted recommendations. Conduct pharmacokinetic and anticoagulation dosing and monitoring. Engage in medication-use review, quality assurance, and drug utilization evaluations. Prepare, sterilize, label, and dispense medications in compliance with regulatory standards. Manage medication inventory, controlled substances, emergency medications, and documentation. Maintain compliance with federal, state, and accreditation standards. Participate in staff education, orientation, and interdepartmental communication to promote quality improvement.

Qualifications:

Active pharmacist license applicable to this assignment. Hospital or inpatient pharmacy experience preferred. Proficiency with EPIC electronic medical records system. Strong clinical, communication, and independent working skills in an LTACH environment. Experience in medication reconciliation, discharge medication management, antimicrobial stewardship, pharmacokinetic dosing, and anticoagulation monitoring.
