Position Overview:
The Transition of Care Pharmacy Technician performs pharmacy-related functions under the supervision of a licensed pharmacist and in accordance with departmental policies and procedures to support safe and effective medication management throughout the continuum of care. This position requires a strong foundation in pharmacy calculations, basic pharmacology, health-system pharmacy operations, pharmacy law, and interpersonal communication. The Transition of Care Pharmacy Technician is responsible for obtaining and documenting accurate medication histories for patients upon admission by interviewing patients, family members, caregivers, pharmacies, physician offices, and other available sources. This role supports medication reconciliation efforts by identifying and documenting home medication regimens, medication discrepancies, and potential medication-related concerns within the electronic medical record (EMR). The technician works collaboratively with pharmacists, physicians, nurses, and other healthcare professionals to promote medication safety, improve transitions of care, and optimize patient outcomes. Complex medication issues, discrepancies, and safety concerns are promptly escalated to the pharmacist for review and intervention.

Pay Range:
The pay range for this position is $18.90 - 28.34 per hour.
