Job Description
Overview We are seeking a dedicated Outpatient PT to join our team. The ideal candidate will have a strong background in physical therapy with experience in neurological conditions, orthopedics, and geriatrics. The position offers an opportunity to work in a dynamic environment focused on improving patient mobility and quality of life through evidence-based practices. Responsibilities Conduct thorough assessments of patients' physical conditions, including physiology and anatomy evaluations Implement and follow individualized care plans tailored to patient needs Collaborate with multidisciplinary teams to coordinate patient care and optimize recovery outcomes Educate patients and their families on exercises, mobility techniques, and injury prevention strategies Document treatment progress accurately using EMR systems and maintain detailed patient records Monitor patient progress and modify treatment plans as necessary to achieve optimal results Stay current with advancements in physical therapy techniques and medical terminology relevant to outpatient care Skills Strong knowledge of physiology, anatomy, and medical terminology relevant to physical therapy practice Experience in outpatient clinics Proficiency with EMR systems for documentation and patient management Skilled in discharge planning and developing effective care plans for diverse patient populations Expertise in physical therapy for neurological conditions Ability to work with geriatric, orthopedic, neurologic, and medically complex patients Excellent patient care skills with a compassionate approach toward rehabilitation goals Strong communication skills for effective collaboration with the team This role offers a rewarding opportunity for licensed physical therapists committed to enhancing patient recovery through comprehensive outpatient services.
