In this role, you will deliver personalized rehabilitative care to patients across various settings, including outpatient clinics, hospitals, and home health environments. Responsibilities Assess patients' physical conditions by reviewing medical histories and conducting comprehensive examinations focused on physiology, anatomy, and functional mobility. Develop and implement individualized treatment plans that align with clinical standards and patient goals, incorporating techniques such as trigger point therapy and other physiotherapy modalities. Collaborate closely with healthcare professionals—including physicians, nurses, and occupational therapists—to coordinate care and optimize patient outcomes. Manage clinical documentation accurately in electronic health records (EHR) systems, ensuring compliance with medical terminology and documentation standards. Provide discharge planning and education to patients and their families to support ongoing care at home or in community settings. Monitor patient progress regularly, adjusting treatment plans as needed to maximize rehabilitation effectiveness for conditions such as orthopedics, neurology, pediatrics, or geriatrics. Engage in outpatient rehabilitation center activities, outpatient clinic visits, inpatient care, or home health services to deliver consistent patient care across diverse environments. Qualifications Valid state licensure or certification as a licensed Physical Therapist. Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, inpatient units, or home health settings. Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards. Experience working with geriatric patients and familiarity with geriatrics-specific care strategies. Demonstrated ability to develop effective treatment plans for diverse patient populations including pediatrics and neurology cases. Proficiency in electronic health records (EHR) management systems such as EMR platforms. Excellent client communication skills with the ability to educate patients on therapy exercises and discharge instructions effectively. Prior experience in discharge planning, care plan development, and working within outpatient rehabilitation centers is highly desirable.
