In this vital role, you will deliver personalized physiotherapy services directly to patients in their homes, empowering them to regain mobility, reduce pain, and improve overall quality of life. Your expertise will help bridge the gap between inpatient care and community living, ensuring seamless care transitions and fostering independence. Responsibilities Assess patients' physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to develop tailored care plans that meet individual needs. Implement evidence-based physiotherapy techniques such as trigger point therapy, orthopedics, and geriatrics-focused interventions to promote recovery and pain relief. Coordinate discharge planning with healthcare teams to ensure smooth transitions from hospital or inpatient settings to home care. Deliver therapy sessions across various settings including outpatient clinics, hospitals, and patients' homes within the community. Document patient progress meticulously using Electronic Medical Records (EMR) systems, updating care plans regularly and ensuring compliance with medical terminology standards. ( KANTIME) Educate patients and caregivers on exercises, injury prevention, and self-care strategies to support ongoing recovery at home. Collaborate with multidisciplinary teams to optimize patient outcomes through effective communication and integrated care approaches. Skills Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practices. Proven experience in discharge planning, outpatient therapy, inpatient hospital care, and home & community care environments. Expertise in pediatrics, geriatrics, orthopedics, and outpatient therapy modalities such as trigger point therapy. Familiarity with EMR systems for accurate documentation and patient record management. Excellent patient care skills with a compassionate approach tailored to diverse populations including pediatric and geriatric patients. Ability to apply physiotherapy techniques effectively in various settings including home health and clinics.
