Job Description
CenterWell Home Health seeks a Home Health Physical Therapist to deliver high-quality, in-home rehabilitative care. In this role, you will assess patient needs, create individualized treatment plans, and provide hands-on therapy to improve mobility, function, and safety. Collaborate closely with an interdisciplinary team, educate patients and families, and document care accurately.
