Want to work for the Premier Rehabilitation Center in Prescott, AZ? Granite Creek Health and Rehabilitation , is seeking a Physical Therapist (PT) to join our Inpatient therapy team! Responsibilities Provide patient care through physical therapy techniques, utilizing knowledge of physiology, anatomy, and medical terminology. Develop and implement care plans, including discharge planning and acute care for patients of all ages. Collaborate with healthcare professionals to deliver quality care in various settings such as inpatient, outpatient, and home health. Utilize premium skills in pediatrics, trigger point therapy, orthopedics, and geriatrics to address diverse patient needs. Document patient progress accurately in EMR systems to ensure continuity of care. Qualifications Patient care experience Proficiency in medical terminology and anatomy Knowledge of physiology and physical therapy techniques Experience in discharge planning and care plans Familiarity with acute care settings and pediatric patients Ability to work with EMR systems and provide home & community care Previous experience in inpatient, outpatient, and clinic settings Certification in trigger point therapy and experience in orthopedics, geriatrics Strong communication and interpersonal skills Ability to collaborate with a multidisciplinary team Hiring for
