You will evaluate functional needs, develop individualized plans of care and help patients improve mobility, safety and independence after illness, injury, surgery or hospitalization. This role combines autonomous field practice with close collaboration across an interdisciplinary home health team. Responsibilities Complete timely in-home evaluations and reassessments of mobility, strength, balance, gait, pain, transfers, fall risk, home safety, and ability to perform daily activities. Develop, implement, and update an individualized physical therapy plan of care with measurable functional goals, treatment frequency and discharge criteria consistent with provider orders, payer requirements, and agency policy. Deliver skilled interventions such as therapeutic exercise, neuromuscular reeducation, gait and transfer training, balance activities, manual techniques, and instruction in assistive or adaptive equipment when clinically appropriate. Teach patients and caregivers safe mobility, fall prevention, home exercise programs, equipment use, and strategies that support independence and carryover between visits. Document assessments, interventions, progress, care coordination, and discharge outcomes accurately. Complete OASIS activities when assigned, applicable, and permitted by regulation and agency policy. Communicate changes in condition, barriers, safety concerns, and progress to the ordering provider, clinical manager, and interdisciplinary care team; follow escalation and emergency procedures. Delegate appropriate treatment activities and supervise Physical Therapist Assistants and home health aides in accordance with state law, federal requirements, and agency policy. Coordinate equipment and community resources, contribute to case conferences and quality improvement, and plan for safe transition or discharge. Follow infection prevention, privacy, professional ethics, and patient rights requirements in every care setting. Required Qualifications Graduate of a physical therapist education program accredited by the Commission on Accreditation in Physical Therapy Education or otherwise meeting the licensure requirements of the hiring state. Current, unrestricted Physical Therapist license in California by the start date. Current Basic Life Support certification, or ability to obtain it by the start date. Valid driver's license, reliable transportation, and automobile insurance that meet agency and state requirements. Ability to work independently in varied home environments, organize a mobile caseload, and complete electronic clinical documentation on time. Clear, respectful communication with patients, caregivers, providers, and interdisciplinary colleagues. Preferred Qualifications 5 years of physical therapy experience; home health, geriatric, neurologic or orthopedic experience is preferred. Experience with OASIS, Medicare home health requirements and WellSky Home Health EMR. Experience supervising PTAs and coordinating care across disciplines. Spanish language skills.
