Part-Time Job Summary We are seeking a compassionate, licensed Physical Therapist to join our growing home health team. In this role, you will provide one-on-one physical therapy services to patients in their homes, helping them regain mobility, improve function, and enhance their quality of life. This position offers a flexible schedule, competitive pay, and the opportunity to make a meaningful impact in patients' lives. Responsibilities Perform comprehensive physical therapy evaluations. Develop individualized treatment plans based on patient needs and physician orders. Provide skilled therapeutic interventions in patients' homes. Monitor patient progress and modify treatment plans as appropriate. Educate patients and caregivers on exercises, safety, and home programs. Complete accurate and timely clinical documentation. Communicate effectively with physicians, nurses, and the interdisciplinary care team. Maintain compliance with all home health regulations and company policies. Qualifications Current Physical Therapist (PT) license in the state of practice. Graduate of an accredited Physical Therapy program. Current CPR certification. Valid driver's license, reliable transportation, and current auto insurance. Home health experience preferred but not required. Strong communication, organization, and time management skills.
