In this role, you will evaluate patients, develop and implement individualized treatment plans, and document progress in accordance with professional and payer standards. You'll collaborate closely with physicians, nurses, and rehab teams to optimize functional outcomes, educate patients and families, and ensure safe discharges. Responsibilities Perform comprehensive physical therapy evaluations and develop individualized plans of care. Provide evidence-based therapeutic interventions, including manual therapy and therapeutic exercise. Document assessments, treatments, and progress in the EMR to meet regulatory and payer standards. Collaborate with physicians, nurses, and rehab team members to coordinate patient care. Educate patients and families on home exercise programs, safety, and functional goals. Adjust treatment plans based on patient response and objective outcome measures. Ensure compliance with state practice acts, professional standards, and facility policies. Participate in orientation to new clinical settings and adapt quickly to diverse patient populations. Communicate regularly with recruiter and leadership regarding assignments, preferences, and goals. Contribute to a clinician-first, integrity-driven culture focused on quality patient outcomes.
