Home Health Physical Therapist Home Health Cerritos, CA Job Details PRN | Contract | Per diem $110,000 - $157,000 a year 2 hours ago Qualifications CPR Certification Physical Therapy BLS Certification Driver's License Driving Physical Therapy License Full Job Description East West Home Health is seeking a dedicated, compassionate, and autonomous Physical Therapist (PT) to join our interdisciplinary clinical team. In this role, you will provide comprehensive rehabilitative care to patients in the comfort and safety of their own homes. Our therapy services focus heavily on restoring mobility, reducing pain, preventing falls, and maximizing functional independence. You will work closely with our Director of Patient Care Services (DPCS), nursing staff, and attending physicians to implement and adjust individualized plans of care that drive measurable patient outcomes.
Key Responsibilities Initial & Ongoing Assessments:
Conduct thorough initial physical therapy evaluations, re-assessments, and discharge planning in accordance with physician orders and agency policies.
Plan of Care Development:
Establish functional, measurable goals based on patient assessments and implement specific physical therapy treatments (including therapeutic exercises, gait training, transfer training, and prosthetic/orthotic training).
OASIS Accuracy:
Accurately score and document objective functional metrics (specifically
OASIS GG
items ) to track patient progress and support agency QAPI quality initiatives.
Interdisciplinary Collaboration:
Participate in care coordination, case conferences, and clinical updates with RN Case Managers, Occupational Therapists, Speech Therapists, and Certified Home Health Aides (CHHAs).
Patient & Caregiver Education:
Instruct patients, family members, and caregivers in safe transfer techniques, home exercise programs, and the proper use of assistive devices (walkers, canes, wheelchairs).
Timely Documentation:
Complete and submit clinical evaluation notes, progress tracking, and verbal orders via the electronic medical record (EMR) system within the agency's mandated 24-to-48-hour turnaround window .
Job Requirements & Qualifications Licensure:
Current, active license as a Physical Therapist (PT) issued by the Physical Therapy Board of California (PTBC).
Education:
Graduate of an accredited physical therapy curriculum approved by the Commission on Accreditation in Physical Therapy Education (CAPTE).
Experience:
Minimum of 1 year of clinical experience as a Physical Therapist required; home health experience is preferred but not required (comprehensive onboarding provided).
Health & Safety Clearances:
Valid pre-employment physical exam (completed within the last 12 months). Current Tuberculosis (TB) screening clearance (2-step PPD, blood test, or chest X-ray). Current CPR/BLS Certification for Healthcare Providers.
Background & Transport:
Must pass a criminal background check and clear monthly OIG/GSA exclusion screenings. Valid California Driver's License, clean driving record, and an operational vehicle with current automobile insurance.
