•Clinical Leadership:
Perform comprehensive evaluations and develop physician-approved treatment plans tailored to each resident's recovery journey. •Team Supervision:
Provide direct patient care and expert supervision of Physical Therapy Assistants (PTAs) in accordance with California State Practice Acts. •Interdisciplinary Teamwork:
Collaborate with the medical and nursing teams to communicate patient progress, goals, and discharge planning. •Resident Advocacy:
Lead discharge planning and recommend necessary durable medical equipment (DME) to ensure a safe transition home. •Education:
Provide vital education to nursing staff, patients, and families regarding long-term maintenance and caregiver safety. •Documentation Excellence:
