JOB DETAILS
This Home Health Physical Therapist role is based in Culver City, California, a centrally located and highly desirable community in the Westside region of Los Angeles. In this position, you will provide one-on-one, evidence-based physical therapy services to patients in their homes throughout Culver City and surrounding communities. Typical patients include older adults aging in place, individuals recovering from orthopedic surgeries, patients with neurologic conditions, and those managing complex chronic illnesses who benefit from restorative and maintenance therapy in their home environment. You will perform comprehensive evaluations, develop individualized plans of care, and implement interventions focused on improving functional mobility, balance, strength, endurance, and safety in the home. A typical day may include traveling between several patient homes within a defined geographic territory, conducting initial assessments, revisits, and discharge assessments, and collaborating closely with an interdisciplinary team. Responsibilities include evaluating home safety, recommending adaptive equipment or environmental modifications, educating patients and caregivers, and implementing fall prevention and energy conservation strategies. You will document visits and progress using an electronic medical record system, completing notes promptly and accurately to support high-quality care and regulatory compliance. Patient caseload is focused on manageable visit volumes that allow time for thorough interventions and patient education. Visit frequency and caseload mix are tailored to your experience and the needs of the local community, with an emphasis on delivering attentive, patient-centered care rather than high-volume throughput. You can expect to work closely with home health nurses, occupational therapists, speech-language pathologists, medical social workers, and home health aides to coordinate care plans and achieve patient goals. Shifts typically follow a daytime schedule, with flexibility to organize your day around patient availability and travel routes. Some weekend or occasional evening visits may be requested depending on agency needs and patient preferences, but the emphasis is on creating a sustainable schedule that supports consistency in care. The role is ideal for clinicians who value autonomy and enjoy working in varied home environments, while still benefiting from the support of a collaborative, multidisciplinary team.
