  • PACE Program Pay Range:
    $42
  • 55/hr Requirements to Apply Bachelor's degree in Physical Therapy from a CMS-approved institution Active California Physical Therapy license Minimum 1 year experience working with frail or elderly populations Valid California driver's license, personal vehicle, and auto insurance CPR and First Aid certification (within 6 months of hire) Physical Therapist Job in Humboldt County, CA

  • Community-Based, Mission-Driven Care Job Overview We are seeking a Physical Therapist to provide participant-centered therapy services within a local PACE program.
This role supports older adults through assessment, rehabilitation, and coordinated interdisciplinary care, with services delivered across clinic settings and directly in participants' homes. The ideal candidate is flexible, collaborative, and committed to improving mobility, safety, independence, and quality of life through evidence-based physical therapy.
