This energetic position offers the opportunity to work in our home health environments—delivering personalized care that makes a real difference. You will develop comprehensive treatment plans, collaborate with multidisciplinary healthcare professionals, and utilize your expertise to guide patients through their rehabilitation journeys. Duties Assess patients' physical conditions by conducting thorough evaluations of mobility, strength, and functional abilities. Develop and implement individualized treatment plans tailored to the patient's needs, goals, and medical history. Deliver evidence-based physiotherapy interventions including manual therapy, trigger point therapy, therapeutic exercises, and modalities to promote recovery. Monitor patient progress regularly, adjusting treatment strategies to optimize outcomes. Document all patient interactions accurately using electronic health records (EHR) systems while adhering to clinical documentation standards. Collaborate closely with physicians, nurses, and other healthcare providers to coordinate care plans effectively. Assist with discharge planning by evaluating patient readiness for independence or community care settings. Work with diverse patient populations including geriatrics, pediatrics, neurology cases, orthopedics, and outpatient rehabilitation clients. Educate patients and their families on treatment routines, injury prevention techniques, and home exercise programs to support ongoing recovery. Requirements Valid license or certification as a licensed Physical Therapist in the relevant jurisdiction. Proven experience practicing in a physical therapy role within inpatient hospital settings, outpatient clinics, or home health environments. Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards. Experience working with geriatric patients and familiarity with outpatient rehabilitation center protocols. Ability to develop effective treatment plans based on comprehensive assessments and patient needs. Proficiency in managing electronic health records (EHR) or electronic medical records (EMR) systems for accurate documentation. Excellent client communication skills with an empathetic approach toward patient care. Knowledge of physiotherapy techniques including trigger point therapy and neurology-based interventions is preferred. Prior experience in discharge planning, acute care settings, or hospital environments is highly valued. Ability to work collaboratively within multidisciplinary teams in outpatient or inpatient settings while maintaining high standards of clinical practice.
