La Mesa, CA We are looking for a dedicated and compassionate Physical Therapist who is committed to providing quality rehabilitation services and helping our residents achieve their highest level of independence and function.Responsibilities:
- Perform initial and ongoing physical therapy evaluations.
- Develop and execute personalized treatment programs.
- Document treatment and progress per facility and regulatory requirements.
- Collaborate closely with nursing, rehab, and medical staff to ensure continuity of care.
Qualifications:
- Licensed Physical Therapist (PT) in the state of CA.
- Graduate of an accredited Physical Therapy program.
- Current BLS/CPR certification required.
- Prior experience in a skilled nursing setting preferred.
