In this vital role, you will assess, plan, and implement personalized physiotherapy programs across diverse settings, including inpatient hospital units, outpatient clinics, home health environments, and community care. Responsibilities Conduct comprehensive assessments of patients' physical conditions, utilizing your knowledge of physiology, anatomy, and medical terminology to develop tailored care plans. Deliver evidence-based physiotherapy interventions such as therapeutic exercises, trigger point therapy, and mobility training to promote recovery and improve function. Manage discharge planning processes by coordinating with multidisciplinary teams to ensure seamless transitions from hospital to home or community settings. Provide specialized care for diverse populations including pediatrics, geriatrics, orthopedics, and patients requiring acute or outpatient care. Document patient progress accurately within electronic medical record (EMR) systems while maintaining detailed treatment records and progress notes. Educate patients and their families on injury prevention, self-care techniques, and home exercise programs to foster long-term wellness. Collaborate with healthcare professionals in inpatient hospital units, outpatient clinics, home health services, and community programs to optimize patient outcomes. Qualifications Bachelor's or Master's degree in Physiotherapy or an equivalent credential from an accredited program. Valid licensure or registration as a licensed Physical Therapist in the relevant jurisdiction. Proven experience in inpatient hospital settings, outpatient clinics, or home & community care environments. Strong knowledge of physiology, anatomy, medical terminology, and therapeutic techniques such as trigger point therapy. Familiarity with EMR systems for documentation and patient management. Experience with discharge planning and developing comprehensive care plans for diverse patient populations. Excellent communication skills to effectively educate patients and collaborate with multidisciplinary teams.
