Job Description

Job Summary:
To provide skilled physical therapy services, case management, and consultation to inpatients, skilled nursing facility (SNF) residents, and outpatients across the continuum of care. Evaluates, plans, and delivers individualized treatment programs, and collaborates with physicians, nursing, and interdisciplinary team members to support optimal patient outcomes. Able to work in settings requiring very independent functioning.
Minimum Qualifications:
Education:
High School Diploma or equivalent. Graduation from a physical therapy curriculum acceptable to the Commission on Accreditation in Education of the American Physical Therapy Association and approved by the California State Board of Medical Quality Assurance.
Experience:
2 years of experience. License oir
Certification:
A current California Registered Physical Therapist license. BLS (Basic Life Support) through the American Heart Association are required.
Other Qualifications Required:
Ability to communicate clearly in both oral and written forms using the English language. Ability to respond to common inquiries or complaints from residents, patients, physicians, peers, regulatory agencies, or members of the business community. Ability to work independently.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Conduct evaluations and plan, organize and administer treatment programs and goals. Responsible for the quarterly screening of SNF residents. Responsible for assessment and referral of residents for Restorative Nursing Program. Responsible for training, delegating and assigning treatment procedures to the physical therapy aide and physical therapy assistant. Participate as the physical therapy representative on interdisciplinary review team. Work to promote comprehensive long-range care for patients with a wide variety of disabilities. Responsible for coordination of department services with SNF services. Supervises the physical therapy aide and physical therapy assistant who provide physical therapy services. Provides In-services to hospital employees on PT related issues. Represents and promotes the department in the community and to referring physicians. Documents evaluations, progress notes, and treatment plans accurately and in a timely manner in accordance with department, payer, and regulatory standards.
  • Educates patients and families on home exercise programs, safety precautions, and use of assistive or adaptive equipment.
  • Participates in discharge planning and makes recommendations for follow-up care, equipment needs, or referrals to other services.
  • Follows infection control, safety, and body mechanics standards to protect patients, self, and coworkers.
  • Maintains current knowledge of evidence-based practice, treatment techniques, and regulatory changes affecting physical therapy practice.
Accountable for department compliance with all State and Federal regulations.
