Plans and provides patient, family, peer education and coordinates services with other health cares professionals including interaction with physicians responsible for the care of the patient. Follows an individualized performance development plan to maintain current knowledge and be competent in critical areas and support program components. Promote hospital goals and objectives. Required Qualifications:

Education to meet certification, license or registration requirement.

California Physical Therapist License Upon hire: National Provider BLS - American Heart Association 1 year of experience as a practicing therapist.
