Key Responsibilities:

Provide high-quality physical therapy services in patients' homes. Develop and implement personalized treatment plans. Assess patients' progress and adjust treatments as needed. Collaborate with other healthcare professionals to ensure comprehensive care. Maintain accurate and timely documentation of patient progress.

Requirements:

Valid Physical Therapist license (or eligibility) in California. Proven experience in home health or similar settings. Strong clinical skills and patient-focused approach. Excellent communication and interpersonal skills. Ability to work independently and manage a flexible schedule.
