$60-$65 Per Hour We are actively seeking a compassionate and dedicated Physical Therapist (PT) to join a Skilled Nursing Facility in Novato, CA through Therapy HCR. Key Responsibilities Evaluate residents and develop individualized treatment plans. Provide skilled physical therapy services to improve mobility, strength, balance, endurance, and functional independence. Assess patient progress and modify treatment plans as needed. Collaborate with interdisciplinary team members to ensure optimal patient outcomes. Educate patients, families, and caregivers on treatment goals, home exercise programs, and safety techniques. Maintain accurate and timely documentation in compliance with facility and regulatory standards. Participate in care planning and discharge planning activities. Qualifications Active California Physical Therapist License. Graduate of an accredited Physical Therapy program. SNF experience preferred, but not required. Strong clinical, communication, and documentation skills. Ability to work effectively in a patient-centered, team-oriented environment.
