Home healthcare- hospice Responsibilities Prioritize start of care visits and manage scheduling to ensure timely initiation of home health services Provide clinical supervision for Physical Therapy Assistants as assigned Complete start of care visits with next-day reporting Document clinical notes and other required documentation within 48 hours of each visit Maintain timely, professional communication with supervisor, intake, and office; respond during normal business hours Perform patient care within a home health framework and utilize the documented system (Kinnser) for ongoing records; equipment issued includes iPad and Verizon One Talk Requirements Active California Physical Therapist license At least 1 year of Home Health experience Ability to perform Start of Care visits and associated documentation Proficiency with the Kinnser documentation system or willingness to learn Must be able to work in a home health setting and support territory needs as assigned Benefits and Support Equipment provided: iPad, Verizon One Talk Mileage reimbursement: $0.
