Responsibilities Conduct thorough evaluations of patients' physical status, including mobility, strength, and functional abilities, utilizing your expertise in physiology and anatomy. Develop and implement individualized care plans tailored to each patient's needs, incorporating techniques such as trigger point therapy and orthopedics when appropriate. Deliver hands-on physiotherapy interventions across various settings including home health visits, outpatient clinics, hospitals, and inpatient units. Manage discharge planning processes by coordinating with healthcare teams to ensure continuity of care post-treatment. Educate patients and their families on exercises, injury prevention strategies, and lifestyle modifications to promote long-term health. Document all patient interactions accurately within electronic medical record (EMR) systems while maintaining compliance with healthcare regulations. Collaborate with multidisciplinary teams to optimize patient outcomes through effective communication and care coordination. Qualifications Valid licensure as a Physical Therapist with current registration in the relevant jurisdiction. Proven experience in home & community care environments with a strong foundation in physiotherapy techniques. Knowledge of acute care, inpatient hospital experience, outpatient therapy, and clinic-based practices. Familiarity with discharge planning procedures and care plan development for diverse populations including pediatrics and geriatrics. Strong understanding of medical terminology along with anatomy and physiology principles essential for effective treatment planning. Experience working with EMR systems for documentation and patient record management. Ability to perform detailed assessments involving orthopedics, trigger point therapy, and other specialized modalities.
