Our facility is conveniently situated in Pleasant Hill, offering an easy and accessible commute for local professionals. Your Clinical Impact As a Physical Therapist at Pleasant Hill, you will lead the charge in resident recovery through:
Evidence-Based Leadership:
Developing comprehensive treatment plans and securing physician approvals to restore resident mobility.
Direct Intervention:
Delivering high-quality therapeutic care tailored to personalized resident goals.
Mentorship:
Providing clinical supervision for Physical Therapy Assistants (PTAs) and guiding our restorative nursing team.
Integrated Care:
Playing a pivotal role in Patient Care Conferences and Utilization Reviews to ensure holistic care.
Documentation Excellence:
Maintaining meticulous records in full compliance with the PT Board and third-party payer requirements.
Discharge Empowerment:
Facilitating safe transitions home through caregiver training and recommendations for essential medical equipment (walkers, braces, etc.).

Qualifications Licensure:
A current and valid Physical Therapist (PT) License in the state of California.
