Full-time, Part-time, PRN / Per diem We are a growing Medicare-certified home health agency looking for a licensed Physical Therapist, PT to provide skilled therapy services to patients in their homes throughout the Inland Empire and Orange County . We are open to full-time, part-time, and PRN/per diem candidates depending on availability, experience, and coverage area. This is a great opportunity for a Physical Therapist who wants flexibility, competitive per-visit pay, and a supportive office team that helps coordinate patient care so you can focus on your patients. Compensation is paid per completed visit. Indeed may display the rate as hourly for posting purposes, but this position is compensated on a per-visit basis. Responsibilities Provide skilled physical therapy services to patients in the home setting Complete evaluations, reassessments, routine visits, and discharge visits as assigned Develop and update individualized therapy plans of care Educate patients and caregivers on exercises, safety, mobility, transfers, fall prevention, and home exercise programs Communicate with physicians, nurses, office staff, patients, caregivers, and other disciplines Supervise Physical Therapist Assistant services when applicable Monitor patient progress and update treatment goals as appropriate Complete documentation accurately and timely in accordance with home health requirements Maintain professional, compassionate, and patient-centered care Requirements Active California Physical Therapist license Current CPR/BLS certification Home health experience preferred, but not required Reliable transportation Valid driver's license Ability to travel within assigned service area Strong communication and documentation skills Ability to work independently in the home setting Dependable, organized, and patient-focused Schedule Full-time, part-time, and PRN/per diem available Flexible scheduling Monday to Friday availability preferred Weekend availability welcome Visits assigned based on your preferred coverage area when possible Coverage areas include the Inland Empire and Orange County Compensation $60 to $150 per completed visit Pay varies based on visit type, experience, availability, and coverage area. Additional compensation may be available for weekend visits, high-priority visits, or difficult-to-cover areas.
