Your Role:
The Physical Therapist is responsible for evaluating, planning, and carrying out individualized treatment programs for home health patients while coordinating care with the broader rehabilitation and clinical team.Key Responsibilities:
- Provide effective patient care using approved therapy techniques and procedures
- Evaluate patient condition and establish measurable treatment goals
- Develop and implement individualized treatment plans
- Supervise and coordinate care with PTAs and HHAs
- Document patient progress, updates, and treatment notes accurately and on time
- Educate patients and families on exercises, mobility, and use of adaptive equipment
- Evaluate home environments and recommend modifications to improve safety and independence
- Ensure all services meet HIPAA standards for confidentiality
- Coordinate with nursing, social work, and other clinical staff to ensure comprehensive care
Qualifications Qualifications:
- Active Physical Therapist (PT) license in CA
- Minimum 1 year of PT experience preferred
- Physical exam within the last 6 months
- Current TB test (within 12 months) and CPR certification
- Valid driver's license and reliable transportation
