Role Description This is a full-time home health role for a Physical Therapist at CareMo Home Health. The Physical Therapist will be responsible for creating customized treatment plans, conducting thorough patient evaluations, maintaining accurate patient records, and collaborating with interdisciplinary care teams. The Physical Therapist will also be responsible for providing in-home care and rehabilitation services to patients. Qualifications Expertise in Physical Therapy Experience in Rehabilitation and Patient Care Familiarity with Home Care practices and protocols Strong interpersonal and communication skills Ability to create and maintain accurate patient records Interest in continuing education and staying up-to-date with industry advancements Willingness to work collaboratively with interdisciplinary care teams Ability to adapt to changing patient needs and requirements Master's degree in Physical Therapy Doctorate degree in Physical Therapy Licensure as a Physical Therapist in the state of California Certification in specialized areas such as geriatrics or pediatrics is a plus
