Job Description

Job Summary:
We are looking for a dedicated and skilled Physical Therapist (PT) to join our Home Health team. The Home Health PT evaluates patients in their homes, develops personalized care plans, and provides hands-on therapy to help improve mobility, strength, and overall quality of life. The ideal candidate will be passionate about helping patients achieve their highest level of independence in the comfort of their own homes.
Key Responsibilities:
Conduct comprehensive physical therapy evaluations in the patient's home environment. Develop, implement, and regularly update individualized therapy plans based on patient goals and progress. Provide therapeutic interventions to improve mobility, balance, strength, and functional ability. Educate patients and caregivers on exercises, techniques, and safety measures to promote independence. Collaborate with physicians, nurses, and other healthcare team members to coordinate patient care. Accurately document assessments, treatments, and patient progress in a timely manner. Monitor changes in patient conditions and communicate effectively with the care team. Ensure compliance with all regulatory, licensing, and agency standards.
Qualifications:
Graduate of an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate in Physical Therapy). Current and active Physical Therapist license in CA. Minimum of [1] year clinical experience (Home Health experience preferred). Current CPR certification. Valid driver's license and reliable transportation. Strong interpersonal, communication, and clinical skills. Ability to work independently and manage a flexible caseload.
Physical Requirements:
Ability to travel to patient homes. Ability to perform physical tasks such as standing, walking, bending, lifting, and transferring patients as needed.

Pay:
$95.00 - $120.00 per hour
