Job Description

South Pacific Rehab Services is seeking a PTA to join our collaborative rehab team serving skilled nursing and post-acute care facilities across Hawaii. In this role, you will implement individualized physical therapy treatment plans under the direction of a licensed PT, focusing on maximizing functional independence, safety, and quality of life. Responsibilities include therapeutic exercises, gait and balance training, patient and family education, accurate documentation, and close communication with the interdisciplinary care team. We offer mentorship, professional development, and diverse clinical experiences in a supportive, patient-centered environment. Responsibilities Implement PT treatment plans under supervision of a licensed PTProvide therapeutic exercises, gait, balance, and functional mobility training Monitor patient progress and communicate changes to the PT and care team Educate patients and families on exercises, safety, and functional goals Accurately document treatments, progress notes, and outcomes in EMREnsure safe treatment environments and follow infection control protocols Collaborate with nursing, rehab, and medical teams on care planning Participate in care conferences and contribute to discharge planning Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Physical therapy documentation (EMR) Patient and family education Safety and fall-prevention techniques Post-acute and SNF rehab protocols Therapeutic modalities application Interdisciplinary team collaboration Treatment plan progression and monitoring
