Job Description
As a Home Health Physical Therapist , you'll practice at the very top of your license, delivering skilled, one-on-one care directly in patients' homes. No two days are the same, and every visit is an opportunity to create meaningful progress for someone in their most comfortable environment. You'll also lead and support Physical Therapist Assistants (PTAs), ensuring that care is delivered safely, effectively, and aligned with each patient's goals. Working closely with an interdisciplinary team, you'll continuously adjust treatment plans to drive the best possible outcomes. In this role, you will: Conduct comprehensive evaluations and establish clinical diagnoses to guide care Develop and implement individualized treatment programs focused on real progress and function Reassess patients and modify care plans to maximize recovery and results Apply infection control and safety standards in all patient interactions Maintain accurate, timely, and compliant documentation Communicate and collaborate with patients, families, and care teams Supervise PTAs to ensure quality and continuity of care This role offers autonomy, flexibility, and the opportunity to truly impact lives—helping patients regain independence and improve their quality of life in the comfort of their own homes. Requirements Successful completion of accredited physical therapy program and state licensure exam. Valid Physical Therapy license 1 year of home health experience preferred CPR certification Valid Driver's license Valid Auto Insurance HomeCare HomeBase experience preferred Physical Demands Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
