As a Home Health Physical Therapist , you'll practice at the very top of your license, delivering skilled, one-on-one care directly in patients' homes. No two days are the same, and every visit is an opportunity to create meaningful progress for someone in their most comfortable environment. You'll also lead and support Physical Therapist Assistants (PTAs), ensuring that care is delivered safely, effectively, and aligned with each patient's goals. Working closely with an interdisciplinary team, you'll continuously adjust treatment plans to drive the best possible outcomes. In this role, you will: Conduct comprehensive evaluations and establish clinical diagnoses to guide care Develop and implement individualized treatment programs focused on real progress and function Reassess patients and modify care plans to maximize recovery and results Apply infection control and safety standards in all patient interactions Maintain accurate, timely, and compliant documentation Communicate and collaborate with patients, families, and care teams Supervise PTAs to ensure quality and continuity of care This role offers autonomy, flexibility, and the opportunity to truly impact lives—helping patients regain independence and improve their quality of life in the comfort of their own homes. Requirements Successful completion of accredited physical therapy program and state licensure exam. Pay-Day The Company operates a bi-weekly payroll process, paid on Tuesdays (10 days in arrears), normally 26 times annually. If one of the paydays falls on a Tuesday, Company Holiday, the the employee will be paid on Monday. Working Hours To meet the needs of our patients and clients, employee work hours may vary and will be set by each employee's supervisor, manager, and operating license documentation. Employees should consult with their supervisors regarding their schedules and time recording procedures. Choice Health at Home, the Company, is open Monday through Friday, office hours are 8:00 am to 5:00 pm (CST). Mileage The Company recognizes the use of personal vehicles by personnel for business purposes as part of their employment. The company will reimburse the eligible employee $0.50 per mile to cover personal care use, auto insurance, depreciation, and gas. Each employee who is eligible for mileage reimbursement from the Company will be required to track and report mileage accurately and timely each pay period.
