Duties Conduct comprehensive patient assessments to develop tailored care plans that address specific mobility, pain management, and functional goals Deliver evidence-based physiotherapy interventions such as manual therapy, trigger point therapy, and therapeutic exercises to promote healing and restore movement Coordinate discharge planning with multidisciplinary teams to ensure seamless transition from hospital to home or community settings Manage acute care cases by providing immediate interventions in fast-paced hospital environments while adhering to medical terminology and clinical protocols Document patient progress meticulously using electronic medical record (EMR) systems, ensuring accurate and accessible records for ongoing care Educate patients and their families on physiotherapy techniques, injury prevention strategies, and home exercise programs to support long-term wellness Collaborate with healthcare professionals specializing in orthopedics, geriatrics, pediatrics, and outpatient care to optimize treatment outcomes Experience Proven experience working in inpatient hospital settings with a strong understanding of acute care procedures and care plans Demonstrated proficiency in outpatient clinics or home & community care environments with a focus on patient-centered therapy approaches Familiarity with EMR systems for documentation and communication within multidisciplinary teams Knowledge of physiology, anatomy, medical terminology, and specialized therapies such as trigger point therapy and orthopedics Experience managing diverse patient populations including pediatrics and geriatrics with sensitivity to their unique needs Strong background in home health services or outpatient physiotherapy practice preferred Prior involvement in discharge planning processes and familiarity with hospital experience standards is highly valued Embark on a rewarding career where your skills make a tangible difference!
