As a Home Health Physical Therapist, you'll deliver one-on-one patient care while playing a critical leadership role in supervising and guiding Physical Therapist Assistants (PTAs) . Your expertise ensures every patient receives safe, effective, and goal-driven care—every visit, every time. This is your opportunity to practice at the top of your license , with the autonomy to make clinical decisions and the responsibility to oversee the full continuum of therapy care .
Your Impact:
Perform comprehensive evaluations and establish clinical diagnoses Develop and implement individualized treatment plans focused on functional outcomes Directly supervise PTAs , ensuring treatments align with the plan of care and regulatory standards Provide ongoing guidance, mentorship, and clinical oversight to PTAs Conduct reassessments and supervisory visits to ensure patient progress and quality outcomes Maintain accurate, compliant documentation for both your care and PTA services Collaborate closely with patients, families, and the interdisciplinary team Ensure infection control and patient safety standards are consistently followed

What We're Looking For:
Licensed Physical Therapist (PT) Experience supervising PTAs (preferred, but not required) Strong leadership, communication, and clinical decision-making skills Passion for accountability, quality care, and team development Requirements Successful completion of accredited physical therapy program and state licensure exam. Valid Physical Therapy license 1 year of home health experience preferred CPR certification Valid Driver's license Valid Auto Insurance HomeCare HomeBase experience preferred Physical Demands Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods. Must be able to lift, carry, pull, or push up to 100 lbs..
