Our Fort Collins Visits team is looking for a per diem Weekend Physical Therapist! We believe clients heal best at home — and clinicians thrive when they're trusted, valued, and empowered. Please note that prior Medicare Certified Home Health and OASIS documentation experience is strongly preferred as most weekend visits are Start of Care Admissions. You will:

Perform mobility, strength, balance, and safety assessments Develop individualized, evidence-based treatment plans Provide gait training, manual therapy, balance work, and therapeutic exercise Implement skin and wound-prevention strategies Educate clients and families to promote long-term success Document care using our intuitive EMR system Compensation & Schedule Competitive weekly pay Flexible scheduling Paid training Mileage reimbursement or schedule-specific benefits

Pay:
