As a Home Health Physical Therapist, you'll deliver skilled, one-on-one patient care while enjoying the autonomy and flexibility that comes with managing your own schedule and patient relationships. You'll also play an important leadership role by supervising and supporting Physical Therapist Assistants (PTAs), helping ensure care remains safe, effective, and aligned with each patient's individualized goals. What You'll Do Perform comprehensive patient evaluations and establish clinical assessments to guide care planning Develop and implement individualized treatment plans focused on restoring function and achieving measurable outcomes Reassess patient progress and adjust treatment approaches to maximize recovery Deliver evidence-based therapy interventions in the home setting Educate patients and caregivers to promote independence and long-term success Collaborate with physicians and interdisciplinary care teams to coordinate patient care Maintain timely, accurate, and compliant documentation Supervise and mentor Physical Therapist Assistants (PTAs) to support quality outcomes and continuity of care Follow infection prevention and patient safety standards during all visits Qualifications Graduate of an accredited Physical Therapy program Current and unrestricted Physical Therapist license Minimum one (1) year of Home Health experience preferred Current CPR certification Valid driver's license and reliable transportation with current auto insurance Experience with HomeCare HomeBase preferred Physical Requirements Ability to stand, walk, sit, bend, kneel, squat, and climb for extended periods Ability to lift, carry, push, and pull up to 100 pounds as needed for patient care activities Benefits & Perks Medical, Dental & Vision Insurance 401(k) after 3 months of service 100% immediate vesting Choice Health at Home contributes 100% of the first 3% you contribute each pay period Health Savings Account (HSA) Life Insurance Short-Term & Long-Term Disability Insurance Paid Time Off (PTO) and Paid Holidays Mileage reimbursement stipend or company vehicle (based on availability after 90 days of full-time employment and Fleet Program eligibility requirements) Join a team where you can work independently, build meaningful patient relationships, and make an impact every day—one home visit at a time.
