Job Description

La Villa Grande Care Center seeks a compassionate Physical Therapist to provide high-quality rehabilitative care to seniors in our skilled nursing community. In this role, you will assess residents, develop and implement individualized treatment plans, deliver therapeutic interventions to improve mobility, strength, and function, and collaborate closely with nurses, physicians, and rehab team members. You'll document progress, educate residents and families, and support safe discharges. Responsibilities Assess residents' functional status and mobility needs Develop and modify individualized treatment plans Provide therapeutic exercises, gait training, and balance activities Collaborate with nurses, physicians, and interdisciplinary team members Educate residents and families on home exercises and safety Document evaluations, treatments, and progress notes accurately Participate in care conferences and discharge planning Ensure compliance with regulatory and professional standards Promote resident dignity, safety, and quality of life Contribute to a collaborative, team-oriented work culture Required Skills Physical therapy evaluation and treatment planning Gait and balance training Therapeutic exercise prescription Manual therapy techniques Geriatric and post-acute rehabilitation Clinical documentation and EMR use Interdisciplinary care coordination Patient and family education Fall prevention and safety training Regulatory and compliance knowledge (SNF)
