Role:
Join our dedicated team at Samash Holistic Home Care LLC as a Physical Therapist, where you'll play a vital role in enhancing the quality of life for our clients in Bridgeport, CT. This is an exciting opportunity to work in a supportive environment that values holistic approaches to health and wellness.
Responsibilities:
Conduct comprehensive assessments to evaluate patients' physical abilities and needs. Develop personalized treatment plans aimed at achieving individual rehabilitation goals. Implement therapeutic exercises and modalities to enhance mobility and reduce pain. Educate patients and families on treatment strategies and home exercise programs. Monitor progress and adjust treatment plans as necessary to ensure optimal outcomes. Collaborate with other healthcare professionals to provide integrated care. Maintain accurate documentation and records of patient progress and treatment. Stay updated on the latest therapeutic techniques and industry best practices.
Requirements:
Valid state license to practice as a Physical Therapist in Connecticut. Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program. Experience in home health care or outpatient settings is preferred. Strong communication and interpersonal skills to engage with patients and families. Ability to work independently and as part of a collaborative team. Compassionate demeanor with a commitment to patient-centered care. Knowledge of current evidence-based practices in physical therapy. CPR and First Aid certification is a plus.
