  • 60/hour | PRN We are hiring a Physical Therapist (PT) on a PRN basis at Civita Care Center at Cheshire.
Join a collaborative rehabilitation setting where you can provide individualized therapy focused on helping residents regain mobility, build strength, and work toward greater functional independence. What You'll Do Complete physical therapy evaluations and develop individualized plans of care Provide skilled interventions addressing strength, balance, gait, transfers, endurance, and mobility Establish appropriate rehabilitation goals and track resident progress Adjust treatment plans based on clinical findings and response to therapy Educate residents and caregivers on therapeutic exercises and safe mobility strategies Maintain clear and timely clinical documentation Coordinate care with PTAs, nursing, and other members of the interdisciplinary team What We Offer $55
