On-Site A growing outpatient rehabilitation and physical therapy organization is seeking a licensed Physical Therapist to join its collaborative healthcare team. The practice is dedicated to delivering individualized treatment plans focused on improving patient mobility, strength, recovery, and overall quality of life through evidence-based care. Key Responsibilities Evaluate patients and develop customized treatment plans Provide rehabilitation services including therapeutic exercises and manual therapy Address pain management, mobility limitations, and recovery goals Utilize therapy equipment and treatment techniques to support patient progress Maintain accurate patient documentation and progress notes Collaborate with healthcare professionals to ensure optimal patient outcomes Educate patients on home exercise programs and recovery strategies Qualifications Active Physical Therapist state licensure required DPT or Master's degree in Physical Therapy required Strong background in rehabilitation and patient care Experience in outpatient, inpatient, or home health settings preferred Knowledge of physical therapy treatment methods and medical applications Excellent communication and interpersonal skills Ability to work effectively in a team-oriented environment Commitment to ongoing professional development and learning Preferred Experience Home health or outpatient rehabilitation experience Strong patient relationship and care planning skills Experience managing mobility and pain-related conditions
