  • 50/hour West River Civita Care Center is seeking a motivated Physical Therapist (PT) to join our rehabilitation team.
This full-time opportunity is ideal for a clinician who enjoys helping residents regain mobility, improve strength, and return to greater independence in a supportive skilled nursing and post-acute care environment. What You'll Do Perform physical therapy evaluations and develop individualized treatment plans Provide skilled therapy focused on mobility, strength, balance, gait, transfers, and endurance Monitor resident progress and adjust treatment plans based on clinical needs Educate residents and caregivers on exercises, mobility techniques, and safety strategies Collaborate with PTAs, OTs, COTAs, SLPs, nursing, and other members of the interdisciplinary team Complete accurate and timely clinical documentation Why Join Our Team?
